Patient profile and concerns

Age/Sex: 60s / F (Asian)
Follow-up: 2 years post-operation

Main concerns: Prominent nasolabial folds, jowls, midface groove, volume loss, long philtrum, and nasal concerns (wide nasal bone and bulbous alar base).

Objective: Restore facial harmony by lifting sagging tissue, improving midface contour, refining the nose and lip proportions, and restoring youthful volume balance.

Surgeries performed

01. Extended deep plane facelift: Dissection and repositioning of the SMAS and deep tissues were performed to lift the midface, smooth nasolabial folds, and improve jowls.

02. Platysma necklift: Through facelift incisions, the platysma muscle and loose neck skin were tightened, improving both the neckline and jawline definition.

03. Fat grafting: Patient’s own fat was harvested and injected into the cheeks, nasolabial folds, and midface groove to restore volume loss and improve contour.

04. Temple lift: A hidden incision in the hairline elevated the temple and lateral brow region, creating a refreshed periorbital look.

05. Upper and lower blepharoplasty: Excess eyelid skin was removed, with fat repositioning to smooth hollows and reduce under-eye bags, achieving a brighter, more youthful eye appearance.

06. Lip lift: The philtrum length was shortened with an incision under the base of the nose, lifting the upper lip for a more youthful balance of the midface.

07. Rib cartilage rhinoplasty: Rib cartilage grafts were used to refine the nasal structure, narrowing the wide nasal bone and reshaping the bulbous alar base while maintaining natural harmony with facial proportions.

Recovery and outcome

The patient recovered smoothly and resumed daily life with minimal complications. At the 2-year follow-up, her results remain stable: lifted and rejuvenated midface with softened nasolabial folds, natural and youthful jawline and neckline contour, refined nasal shape with harmonious balance, shortened philtrum and improved lip-to-nose ratio, restored volume and smoother skin texture.
