Revision Facelift & Necklift

Patient profile and concerns
Age/Sex: 60s, Caucasian female
From: Australia
Follow-up: 1 month post-operation
Main concerns: Deep nasolabial folds, jowls, and overall skin laxity. She had her first facelift 20 years ago and was ready for another revision to address recurrent aging changes.
Objective: To tighten loose skin, rejuvenate the mid and lower face, smooth deep folds, and restore volume for a refreshed and natural appearance.
Surgeries performed
01. Upper blepharoplasty – Removed excess upper eyelid skin to brighten the eyes and improve eyelid contour.
02. Lower blepharoplasty – Adjusted fat pads and removed excess skin for smoother under-eye areas and a youthful transition.
03. Extended deep plane facelift – Lifted and repositioned the SMAS and deeper tissues to correct jowls, soften nasolabial folds, and restore cheek support.
04. Platysma necklift – Tightened the platysma muscle and removed loose neck skin to achieve a firmer, more defined neckline.
05. Middle neck plication – Performed through a small incision under the chin to reinforce central neck muscles, further improving jawline and neck contour.
06. Temple lift – Elevated the lateral brow and temple area through an incision hidden in the hairline, correcting heaviness and rejuvenating the upper face.
07. Fat grafting – Patient’s own fat was injected into areas of volume loss, including the midface and folds, to restore youthful contour and softness.
Recovery and outcome
At 1 month post-operation, the patient shows significant improvement in skin laxity with lifted cheeks, a smoother jawline, softened nasolabial folds, and brighter eyes. The overall effect is a natural yet refreshed appearance, with swelling expected to continue improving over the coming months.
